Admission and Discharge
Admission
The Children’s Station Admission / intake procedure packets will be handled by the Administrator for initial reading. She will initiate individual plan with each client and she will then determine if the referral packet is complete and compatible with the facility’s program. The process of admission takes a minimum of two days from receiving a referral packet. Feedback is given within 48 hours after seeing or visiting the referred client.
The criteria for placement of children in the program are:

The applicant may or may not be physically challenged, deaf or blind (or both), will have a diagnosis of a developmental disability and may or may not have special needs. Development disability is defined as a disability which originated before an individual attains age 18, continues, or can be expected to continue indefinitely, and constitutes a substantial handicap for such individual, to include mental retardation, cerebral palsy, autism or other neurological disorder.

Applicant may be ambulatory or non-ambulatory. The program is designed to accept children who function in the profound to mild range of functioning.

The program is designed to serve developmentally disabled children who have intermittent, reoccurring special needs and supervision / training in self-care and ADL skills.

Eligible consumers will benefit from an intensive program that addresses the needs of physically challenged children.

Applicants must have a complete referral packet which includes Nursing Assessment, Social History, current Medical Evaluation immunization record (which includes TB clearance, shigella, hepatitis, O & P.)

Party responsible for children ( parents, legal guardian or councilor) should be available for an intake interview prior to the children’s admission to the program.
